Ideal and adjusted body weight calculations only apply if patient is >60" tall.

Creatinine clearance and AUC calculations are applicable only if the serum
creatinine is stable.

Dosing and CrCl are based upon actual SCr entered into calculator. In
many cases it may be more appropriate to use a value of 0.8 or 1
for patients with a serum creatinine below 0.8 mg/dl due to the fact that
the lower values are not reflective of clearance, rather they are the
result of low body muscle mass.
